Essential Job Duties and Responsibilities : Provides on-site clinical counseling to clients (and families when needed). Conduct Psychosocial Assessment, Progress Notes and Psychotherapy Treatment Plan and mental health screenings for all clients. Participate in case conferences, as needed with Eaves and building's care team Develops, coordinates and implements discharge plans in collaboration with patient, patient's family and tx team. Weekly/Bi-Weekly discussion/updates with Director to give/get updates and discuss general happenings._(The above job description is not meant to be inclusive of all duties)_ Qualifications Needed: _ Must _ be licensed in Illinois as an LCSW or Licensed Clinical Psychologist (Psy.D or Ph.D) Experience or interest in working with aging populations.
