Job Description:
Position Summary:
As a Registered Nurse specializing in Geriatric care, you will
provide specialized nursing services to elderly patients, ensuring
their physical and emotional well-being. You will collaborate with
a multidisciplinary healthcare team to develop and implement care
plans tailored to meet the unique needs of geriatric patients. Your
primary goal will be to promote optimal health, independence, and
quality of life for elderly individuals.
Key Responsibilities:
Patient Assessment: Perform comprehensive assessments of geriatric
patients, including physical, cognitive, and psychosocial
evaluations. Identify health concerns, chronic conditions, and
potential risks associated with aging.
Care Planning: Develop individualized care plans based on
assessment findings, patient goals, and evidence-based practices.
Collaborate with the healthcare team, including physicians,
therapists, and social workers, to coordinate care and ensure
holistic treatment approaches.
Medication Management: Administer medications and treatments as
prescribed by physicians. Monitor medication effectiveness,
potential side effects, and interactions. Educate patients and
their families on proper medication usage and potential risks.
Geriatric Health Promotion: Implement health promotion strategies
to enhance overall well-being and prevent common geriatric
conditions such as falls, pressure ulcers, and infections. Provide
education to patients and families on healthy aging, disease
prevention, and self-care techniques.
Chronic Disease Management: Monitor and manage chronic diseases
prevalent in the geriatric population, such as diabetes,
hypertension, heart disease, and dementia. Develop strategies to
promote symptom management, improve functional abilities, and
prevent disease progression.
Collaboration and Communication: Foster effective communication and
collaboration with the healthcare team, patients, families, and
caregivers. Ensure the exchange of vital information, maintain
accurate documentation, and advocate for the needs and preferences
of geriatric patients.
Patient Support: Provide emotional support and comfort to elderly
patients and their families. Assist in addressing end-of-life
issues, palliative care, and advance care planning discussions,
respecting the individual's wishes and cultural beliefs.
Geriatric Rehabilitation: Support and facilitate rehabilitation
programs for geriatric patients recovering from surgeries,
injuries, or illnesses. Coordinate with physical therapists,
occupational therapists, and speech-language pathologists to
optimize functional independence.
Geriatric Mental Health: Assess and address the mental health needs
of geriatric patients, including depression, anxiety, cognitive
impairments, and behavioral disturbances. Collaborate with
psychiatric professionals as needed and employ therapeutic
interventions to enhance mental well-being.
Continual Learning: Stay updated on current geriatric nursing
practices, advancements, and evidence-based research. Attend
educational programs, conferences, and workshops to enhance
professional knowledge and skills in geriatric care.
Qualifications and Requirements:
Valid nursing license and registration as a Registered Nurse (RN)
in the relevant jurisdiction.
Specialization or certification in geriatric nursing is
preferred.
Strong knowledge of geriatric health issues, common conditions, and
evidence-based nursing interventions.
Ability to assess, plan, implement, and evaluate care plans for
geriatric patients.
Excellent communication and interpersonal skills to effectively
interact with patients, families, and healthcare team members.
Compassion, empathy, and patience in dealing with the unique
challenges faced by geriatric patients.
Proficiency in electronic medical records (EMR) systems and
healthcare technology.
Physical stamina and ability to perform tasks that involve
standing, walking, lifting, and bending.
Flexibility to adapt to changing work schedules, including
evenings, weekends, and holidays if required.
