Job Description UW Health is seeking a Nurse Practitioner Clinic Family Practice for a job in MADISON, Wisconsin. Job Description & Requirements - Specialty: Family Practice - Discipline: Nurse Practitioner - Duration: Ongoing - Employment Type: Staff Work Schedule: /n 100% FTE, hours: 8:00 am to 4:30 pm or 8:30 am to 5:00 pm. No weekends, holidays, or call. /n Be part of something remarkable /n As an Advanced Practice Provider, you'll play an integral role in our inpatient and ambulatory care teams by providing high-quality, cost-effective care to patients. /n We are seeking a Nurse Practitioner or Physician Assistant to: /n /n - Conduct chart reviews, see patient follow-up, evaluate patients for pain procedures /n - Perform in-person, phone, telehealth, and emergency in-person consults /n - Coordinate care team with Pain Medical Director or Site Physician /n - Order additional tests and medications as needed /n /n Education: If Nurse Practitioner: Master’s or Doctorate’s degree from accredited Nursing Program. If Physician Assistant: Bachelor’s or Master’s degree from accredited PA Program. /n Licenses and Certifications: /n Minimum – State of Wisconsin NP or PA licensure /n If Nurse Practitioner*: /n /n - Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND /n - Active board certification as a NP in area of clinical practice.
