Job Description:

Crossbridge Hospice is seeking a compassionate and dedicated Hospice Community Liaison to join our team. This role is pivotal in enhancing our mission to provide exceptional end-of-life care and support to patients with life-limiting illnesses and their families. The ideal candidate will be responsible for building relationships with healthcare providers, community organizations, and patients to ensure a seamless transition and understanding of the hospice services we offer. This position requires a blend of empathy, excellent interpersonal skills, and a deep understanding of healthcare services. Act as the primary point of contact between Crossbridge Hospice and community healthcare providers. Educate families and patients about hospice services and the benefits of palliative care. Coordinate with healthcare professionals to facilitate referrals and ensure continuity of care. Participate in community events to raise awareness about hospice care options. Develop and maintain professional relationships with local healthcare facilities, physicians, and community groups. Assist in the development of marketing materials and strategies to promote hospice services. Strong communication and interpersonal skills Ability to work compassionately with individuals facing terminal illnesses Proficiency in CRM software and Microsoft Office Understanding of healthcare systems and palliative care principles Strong organizational and multitasking skills Medical Insurance Dental Insurance Vision Insurance 401k Certified Hospice and Palliative Care Administrator (CHPCA) preferred Valid driver's license and reliable transportation
