Job Description:

Encompass Health is seeking a dedicated Pharmacist to join our healthcare team, specializing in the rehabilitation and home health/hospice services sectors. The ideal candidate will be integral in ensuring the provision of effective medication management and counselling services to patients recovering from significant illnesses or injuries. This role involves collaborating with a multidisciplinary team to design and execute the best medication therapies for our patients, supporting their journey to regain independence and achieve optimal health outcomes. Assess medication orders for appropriateness, checking for drug interactions, dosage, contraindications, and allergies. Provide detailed counseling to patients and their caregivers on medication usage, side effects, interactions, and storage. Collaborate with healthcare professionals to plan, monitor, and review medication regimes. Supervise the preparation and dispensing of medications. Maintain accurate records of patient care, condition, progress, and medication documentation. Provide training to the pharmacy staff and oversee their work for compliance with professional standards. Engage in continuous education to remain abreast of new drugs, technology, and treatment practices. Strong knowledge of pharmacology and other medical disciplines. Excellent communication and interpersonal skills. Attention to detail and precision in handling prescriptions. Ability to work efficiently under pressure. Management and leadership skills. Medical Insurance Dental Insurance Vision Insurance 401k Valid state Pharmacist license. Board Certification in Pharmacotherapy (BCPS) or related field preferred.
